Name it

FormSettings: the form’s name, then Submit.

Field types, grouped

Input for typing, Select for choices, and more.

A name and a phone number

Add choices

Label it, add each choice, then Submit.

Share it

Sharing sets who can see it; Open Link opens it at its own address.

Ready for your customers
