Create a form
Open your team and choose UI & Data, where your forms live. Choose Create a new UI/Form, and a new, empty form appears in the list.
Name it
Open the form, choose FormSettings, type its name, Custom Cake Order, and choose Submit to save it.
Pick a field type
Choose Fields. Field types are grouped: Input for typing, Select for choices, and Action, Display and Object for buttons, text, dates and more.
Add a name and a phone number
Choose Input, then name, then phone. Each is added to the form right away, already labeled, and the phone field checks what people type.
Add choices
Choose Select, then options. Open the new field and choose Settings. Type a label, Cake size; under Field options, type each choice and choose Add; then choose Submit. Do the same for the flavor: vanilla, chocolate, lemon and red velvet.
Add a text field
Choose Input, then text, and label it Message on the cake the same way: Settings, Label, Submit.
Share it
Every form has its own link. Close Fields, choose Sharing to set who can see it and fill it in, then Copy Link or Open Link.
See it as your customers will
Choose Open Link, and the form opens on its own page, the way people filling it in will see it: a name, a phone number, a size, a flavor and the message for the cake. Send them the link, or put it on your website.
