Design it: simple to showpiece
Each design level asks only what it needs, and adds its own price.

A Design step with levels
Open the order form (here the Custom Cake Designer from Price it as they choose). Add a Step field called Design, and a Design level choice, Simple, Decorated or Showpiece, each with a picture.
What each level asks
Add what the levels need: a Message on the cake for every design, Decorations (a multiple select: piped borders, fresh flowers, sugar flowers, gold leaf, a topper), and for a showpiece an Inspiration photo and Notes for the decorator.
Put the step in its place
New fields go at the end, after Your details. Choose Move Field, pick the Design step, and choose Step earlier: the step moves with all its fields.
Show, hide and price
In Form Logic, Write as text: on load, hide the decorations, photo and notes; under on update, add each level’s price with switch (Decorated $40, Showpiece $120); and on change "Design level", show what that level asks for.
Simple
On the Design step, Simple asks only for the message, and the price stays the same.
Decorated
Decorated adds decorations to pick from, and $40.
Showpiece
A showpiece also asks for an inspiration photo and notes for the decorator, at $120 more.
